NOTE:

• A cardholder or cardholder representative may submit this cancellation either online or by fax.
• A reason for cancelling cards must be provided. Select from: Lost / Stolen / Other.
• If you need to (urgently) cancel more than 1 Charge Card please write details in the comments box and provide a reason for each cancellation. 

• A cardholder or cardholder representative may phone 02 9020 2345 to cancel Charge Cards lost or stolen outside Cabcharge office hours (8.30 – 4.30 Monday to Friday, Eastern Standard/Daylight Savings time).
• Please provide cardholder name, company and card number (if available).
• Notification must be confirmed in writing stipulating the card number (Ref. Conditions of Use, Clause 10.1a).
• The account holder will not be liable for transactions raised from lost or stolen cards that are dated after the report of the loss or theft (Ref. Conditions of Use, Clause 10.1b).
