NOTE:

• This is a request for an Application Form to Open Charge Account Facilities with Cabcharge to be sent to you (this is NOT an application form).
• The Application Form will be sent to you.  Please complete (and sign) this form and send the original to Cabcharge with a cheque for the bond (a personal guarantee may also be required).
• The bond is held against payment of the account and is refundable in full after the account is closed (Ref. Conditions of Use, Clause 2). To view a copy of our Conditions of Use please click HERE.
• Upon establishment of an account you can be issued with TAXI eTICKETS and/or personalised Charge Cards. 
• A tax invoice/statement is sent at the end of each billing period (28 days), and is payable in full within 28 days of the issue date.
• Payment of invoice/statements may be made by cheque or EFT (Electronic Funds Transfer).  
• An itemised list of all transactions is sent with each invoice/statement.
• Statement data can be downloaded from the Cabcharge website.
